Orthopedic Surgery Associates is pleased to
introduce a new convenience for our patients. Fill in your
forms and information on line. This saves your time in our
waiting room, and allows you to look up items that you might
not have with you when you come to the office.
Follow these simple steps to complete these forms on line.
- You will require Adobe Acrobat. Click here to download if you do not already have the program. The program is FREE.
- Click on the link for the form. A new window will open for the form.
- Complete the form on line. Use your TAB key, or the mouse to move around the form.
- Print the form to your printer. Bring the forms with you to your next visit. (Make as many copies as you want for your records.)
Financial Agreement - A copy of your financial agreement with us is required.
Patient Information - Your medical history should be prepared carefully. It is vitally important to your treatment.
Disclosure Form - We may find it necessary to communicate your health information with other organizations. This is your authorization for us to do so.
